You can get your PUK code from your network provider. Call them up and give them you IMEI. They will give you a code to enter in your phone.

I'm assuming you entered your pin wrong too many times and you just want a pin unlock?

Because you won't get your phone unlocked in the sim-lock sense with a PUK.
